How You Will Make a Difference: Collaborating with our dynamic team, you will support the patient in the implementation of their personalized Care Management Plan. You will provide ongoing support that will extend beyond routine medical assistance to encompass educating patients on condition management, providing behavioral support, and addressing the emotional challenges associated with chronic conditions. By working closely with patients to set and achieve goals, your efforts will contribute to enhanced self-management outcomes.

As the patient's Health Coach, you will not only educate them on self-care but also offer the empathetic and emotional support of a trusted friend. 

Schedule:

Full Time Days – Tuesday, Wednesday, and Thursday in the office, Monday and Friday remote.

What We Require of You: 

  • Experience: 1+ year of experience in Case Management is preferred but not mandatory. Your passion for making a difference matters most. 
  • Certification as a Medical Assistant, or willingness and ability to gain certification within 6 months of hire.
